12 team 0.5 PPR I am 2-2 and he is 0-4. I was offered this trade in the morning.

I've been really hoping that Karlos will emerge as my weekly RB2 but Forsett and Martin have been showing me life. Can I afford to make this trade assuming Karlos is good to go after being diagnosed with a concussion? The league is pretty tight in standings with 4 teams making the playoffs but it's still early and I do like my team enough to squeeze in.

Marshawn doesn't seem as hurt as reports have hinted so I don't know how much value Rawls has after this week, but I do have #4 waiver priority with Duke Johnson/Antonio Andrews/Christine Michael/Andre Williams/Knile Davis/Bradshaw all on the wire. None of them I really like though tbh besides maybe Christine.

Is it wise to make this trade if I think Dez can really help bolster my WR3/flex spot weeks 11-16? Cooks has been a massive disappointment and I'm not sure how consistent Steve Smith will be for the rest of the season. What should I do about my RB situation if I make the deal?

I would probably go ahead and do that deal. Then I'd drop Rawls for Duke Johnson. I think that would be the best move for you and I think you have enough depth on your team to stash Dez. You may still struggle at the WR position, and you may miss Hankerson, but I think it's a risk I'd be willing to take in this situation.
